What this does is enable you to remove the bolt by first drilling a pilot hole in the center of the stuck bolt as straight and as centered as you can (thats important). Next you thread the extractor into the hole until it makes solid contact with the walls of the bolt. Use a hammer to set it in place in the begining. The extractor is "threaded" in reverse. So as you tighten the thing, it turns counter-clockwise, the direction of removal. Use lots of penetrating oil to help the process, let it soak a while. I've used this tool twice in my life. Once on a carberator stud broken off in the intake manifold and on a lawnmower blade nut. Worked both times.
