I just had some expert help fixing this same problem on my Westy -- which is good, since I am definitely NOT a mechanic, OK? HOWEVER:

Remove the blade assembly and use a wire brush to scrape out all the grooves in the whatchamagigger (cone shaped metal head that turns the blade arm). Make sure all the (aluminum?) dust is out of the grooves, even using a small knife blade if necessary. Apparently, the trick here is to place the blade assembly cleanly right down on top of this head without rotating it even slightly -- as that creates the "dust/shavings" in those grooves you just cleaned out and it won't fasten/sit properly.

Make sure you have a washer that's thick enough (don't ask me how thick, maybe someone else knows), place the blade assembly back on with the wiper exactly where you want it on the windshield, and then tighten the nut back down FIRMLY AND WITHOUT TURNING THE BLADE ASSEMBLY AND WITHOUT BUSTING ANYTHING.....maybe make a lot of small but firm turns of the nut -- not all at once.

I have a friend who swears he never has to adjust his blades once he has done this right. Hope it helps.
