Bus Depot sells a flat forged wrench from EMPI that has ears for hammering the nut off, and a half-inch square hole located (on mine) 9 cm from the 46mm, center-to-center.

The formula to calculate resulting torque with an extension is:

Where: e = effective length of extension (distance between perpendiculars through center of hex and center of drive hole) l = effective length of wrench (center of drive to center of handle Tw = Torque set on wrench Te = Torque delivered to nut

Tw = Te * l / (l + e) or Te = Tw * (l + e) / l

For a 9 cm extension length, a 250# wrench would have to be shorter than 19 cm (effective) or about 7.5 in. For an 18" wrench at 250# you'd need a total extension length of about 8.5 in or 22 cm. Etc.
