I had the same problem last week that I thought also was a very noisy lifter, but quickly diagnosed it as an exhaust leak at #1 cylinder. The metal gasket had disintergrated. As far as your flange, just make sure you have enough material (metal) for the nut & bolt. Use a file to resurface the pitted/corroded surface before fitting it all together with new gaskets. If your nuts & bolts are corroded beyond use of wrenches and sockets, just zip-cut the head of the bolt, and punch it out with a metal punch. It helps greatly to support the back of what ever you are hitting with something heavy (like a 5 lbs hammer) to absorb too much shocking to the rest of your exhaust system. 2 man job.
