Most "Net Nanny" type programs work by using the rating which web site
owners can attach to their site.  Almost all of these have an option to
either allow or block "unrated" sites.  Since most adult sites are
voluntarily rated, and most other sites are not rated, most people set this
to allow unrated sites.
