How long has this adapter been in???  Maybe the cap is blown = (you can check it with an analog volt meter by switching the leads back and forth = between 2 and 4 and looking for needle deflection) or it's possible the the AFM = board is wiped out (no pun intended).  You may be = able to move the board so that the arm will use a different path on the = board.
