How long has this adapter been in??? Maybe the cap is blown (you can check it with an analog volt meter by switching the leads back and forth between 2 and 4 and looking for needle deflection) or it's possible the the AFM board is wiped out (no pun intended). You may be able to move the board so that the arm will use a different path on the board.
